I recently purchase a 01 Touring MDX with approx 65K miles from CarMax. I was offer the extended warranty but turned it down. Now I am having seconds thoughts. Have there been any serious issues with the MDX's with higher mileage (> 65K) other than the reported transmission issues with the earlier models?

I don't think there have been major issues besides the transmission on older MDXs. The EGR valve warranty has been extended. Some have complained of gas tank slosh, weeping mirrors, etc but these don't affect performance.
Do make sure the tranny oiljet fix recall was done.
BTW, was the warranty they offered an AcuraCare one? If not, that might not cover the transmission fully, but may be worth it since the car is out of warranty anyway. |

The warranty was not an AcuraCare warranty and the person that had the MDX before I did took care of the transmission issue.
Maybe I will roll the dice and skip the extended warranty.

I've NEVER lost money on an extended warranty.
My 2001 MDX has 125k miles- I have had no major issues at all, and it has been very reliable. BUT, the repairs under the warranty exceeded the cost of the warranty!
A seat release, an antenna, front shocks, alternator. Misc other stuff.
Extended warranties vary greatly- shop, make sure you know what you are buying. a 100k mile 6 year warranty cost me 1400.
