I tried doing a search on this, but no luck. I hear many ideas, but no specific failure cause.

Design flaw in 01-02 transmissions. Cetain gears were not getting sufficient ATF flow causing excessive heat which resulted in gear failure. oil jet gear kit was made to solve this issue and provide enough ATF flow.

Also note many trans issues were not really trans issues. Many transmissions have been misdiagnosed and replaced when they only really needed a maf sensor, egr valve, etc.

Yes that was the recall. True enough.
It is a bit more complicated. A much more common problem is wearing of the third gear clutch. If the clutch wears, it can not hold. And it slips, most often on a 2-3 shift at high power. Particles of the worn clutch also clog oil passages and internal screens so that ATF can not flow to move internal parts to engage gears when they are supposed to. This more common problem is not solved by the oil jet. Even frequent ATF changes do not prevent failure before 100,000 miles.

The oil jet recall is a recall because it is a safety defect. The overheated gear can break and lock up the transmission. An accident is likely.

The more common clutch problem is not a recall. Not sure why not.

A crappy design bottom line....and it has cost Honda Corporation Millions and continues to this day!
Dealerships suffer
Consumers suffer
Manufacturers suffer

One can HOPE that they learned a lesson! I personally would AVOID any 98 to 03 V6 auto transmission vehicle made by HONDA!

Agreed as a former owner of a 98 V6 Accord and 02 TL. Fortunately, Honda paid for the repairs, but surely reduced my lifelong confidence in Honda. Despite this, I still love their vehicles and my 04 and 06 MDXs transmissions have been flawless. I looked seriously at Lexus, but the price point turned me away. They may advertise similar dollars, but if you similarily equip the RX and MDX, The Lexus costs a lot more.
