I had a high pitched whining sound between 60 and 70 mph in my 2004 MDX but only when pressing the gas. Dealer thought it might be something to do with the throttle and cleaned during 60M mile service. Sound continued and finally they replaced transmission. Still have noise and it is worse. I hear at 35 mph and between 60 and 80, but still disappears when I let off gas.
Luckily I took out extended warranty. Any ideas?

If it doesnt make that noise when you rev the vehicle when parked, my first suspect would be the vtm system

Had a similar noise in my 2004 between 25 and 45 MPH. I took it to the dealer and they said it was the tranny. They replaced it for free since it was still under warranty. Go have them look at it.

He already said he had the trans replaced for free, and the noise still remains.
